Steps to Schedule An Appointment

1. Make sure you qualify!

• You have permanent or temporary residency in the state of Florida.
• You have a qualifying condition diagnosed by a physician.

2. Make an appointment!

• Fill out your information to the right and one of our staff will contact you to make an appointment.

3. Get Certified!

• Bring your identification and medical records to your appointment!
• remember to bring cash for your payment.
• Once you meet our physician, sign the consent form and he determines you are qualified, he will then proceed to enter you into the Medical Marijuana Use Registry.
• You will then recieve an email to confirm your account: pay the state fees, and then you will be mailed your certification card.
• Once you recieve the state issued certification card, you are free to visit a certified dispensary to fill the physician order.
